The Illinois Project for Local Assessment of Needs (IPLAN) is a process that every certified health department in Illinois uses to assess the health of the community and plan programs that address health needs that are specific to the local community. Simply stated, it is a way of determining as a community what the health needs are in our counties and how we can best utilize our resources to improve the health of our citizens.
The IPLAN committee is composed of members of the community from a broad range of fields, including St. Margaret's. The committee's role is to choose what we see as the top three health problems in Bureau and Putnam County, through discussion and reviewing statistics. Then, over the course of the next five years the health department and community partners will work to improve the health of our community in these four areas. Because health needs are identified by a committee of community members, the IPLAN represents a health assessment of our community, by our community.
The objectives and strategies are designed to address the risk factors and contributing factors listed in the Health Problem Analysis Diagram that are within the scope of the IPLAN to address.
