Primary Duties:

The Registered Nurse has the responsibility for the following: Completes all required patient assessments timely and accurately; completes discharge instructions or transfer information completely and accurately; completes all documentation accurately and timely; directs and assists non-licensed personnel; provides appropriate comfort measures and pain management; demonstrates knowledge of all wellness and disease management issues; administers medications accurately and timely; demonstrates competency in patient care procedures and use of pertinent equipment and supplies; executes accurate order entry, transcription of orders and signs off orders; provides patient education; initiates and up dates patient plan of care; and promotes and assures patient safety.

Qualifications: Graduate of an accredited school of nursing required. Current State of Illinois registered nurse license, current CPR certification and any other certification appropriate to specialty required.

Primary Duties:

To provide transportation services of patients, the elderly, the disabled and other passengers in need of transportation to access health care. In doing so, staff will escort ambulatory passengers on and off the vehicle, transport and secure wheel chair passengers, and assist passengers with transportation needs from “door to door”. Para-Transit drivers are directed by the Para-Transit Coordinator.

Qualifications: High school graduate or GED equivalent. Must possess good driving record and be at least 25 years of age, for insurance purposes. Must hold a current valid State of Illinois Driver’s License.
