HOME CARE SERVICES

Prairieland Home Care pulls together a team of professional nurses, certified home health aides and

therapists who visit the patient's home as often as necessary. After contacting Prairieland, the agency will

arrange for an evaluation of the patient's needs and develop an individualized plan of care that is ordered

by the physician. Payment for the service can be made through a number of resources. Prairieland Home

Care is a not-for-profit organization and serves parts of Bureau, LaSalle, Putnam, Marshall and Lee Counties.

HOSPICE

Hospice care supports terminally ill patients and their families for whom curative therapy is no longer possible

or appropriate. Emphasis is put on the management of pain and other symptoms associated with terminal

illness. Requests for hospice care are accepted from the patient, family or physician. St. Margaret's Hospice
