Scope of Responsibility:

St. Margaret’s Health Behavioral Health Case Manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned caseload.  The behavioral health case manager works within an integrated and collaborative care team to coordinate the care of patients across a spectrum of health services, including access to physical health and behavioral health care, as well as social services. The behavioral health case manager also provides a recovery oriented approach to help patients with mental and substance use disorders manage their conditions successfully. This position is a non-exempt position and reports to the Director of Behavioral Health.

Education Requirements:

Bachelor’s degree in Psychology, Social Work, Nursing, or closely related field by an accredited college.

License/Certifications:

Licensed Social Worker or Registered Nurse or eligible for licensure within the State of Illinois preferred, but not required. Certification as a Certified Recovery Support Specialist (CRSS) or eligible to complete CRSS certification requirements within one year of hire.

Special Requirements:

Lived Experience of mental health/substance use recovery. Understanding and implementation of strong clinical and ethical boundaries.  Able to maintain patient confidentiality, respect and empathy.  Must be enthusiastic about providing support and recovery to patients in all stages of recovery.
