Prairieland Home Care, part of the SMP Health System, pulls together a team of professional nurses, certified home health aides and therapists who visit the patient's home as often as necessary. After being contacted, Prairieland will arrange for an evaluation of the patient's needs and develop an individualized plan of care that is ordered by the physician. Payment for the service can be made through a number of resources. Prairieland Home Care is a nonprofit organization and serves parts of Bureau, LaSalle, Putnam, Marshall and Lee Counties.
